I forget exactly how it is played but it starts with everyone sitting in a circle. The first person looks at a card off the top of the deck and chooses to keep it or pass it. I believe the object is to get 3 of a kind(?). Perhaps it is an A, K, Q, J. I may be wrong on this as it has been a while since I played and usually get quite drunk while playing. Anyway, the first person to get three of a kind(?) then reaches for a spoon.
There is one less spoon than the number of players. After the first person grabs a spoon, then everyone else can grab a spoon. The person left without a spoon, removes an article of clothing or drinks. Of course, this relies on how you are playing the game.
